Job Overview: We are seeking dedicated and mission-driven professionals to join our Remote Patient Monitoring Program as Medical Assistants. In this pivotal role, you will contribute significantly to enhancing patient care and satisfaction while collaborating closely with our clinical team to support chronic disease management.
Key Responsibilities
- Enroll patients into the Remote Patient Monitoring (RPM) program and ensure coordinated care throughout their journey.
- Monitor patient compliance through regular check-ins and facilitate care coordination with other medical specialties.
- Collect and document essential patient information, including medical history and vital signs, to ensure comprehensive care.
- Identify care gaps and assist in coordinating treatment with caregivers and healthcare providers.
- Maintain accurate and compliant patient metrics and records in adherence to program guidelines.
- Conduct follow-up outreach to address acute concerns and keep patients informed regarding their care and associated costs.
- Provide clerical support by managing general office duties such as processing communications and maintaining medical records.
- Collaborate within an interdisciplinary team to promote optimal patient outcomes and participate in team discussions.
Required Skills
- Strong organizational abilities with aptitude for multitasking and prioritization.
- Proficient in various computer applications, including EHR systems like Athena, Microsoft Word, and Excel.
- Familiarity with medical terminology, CPT, and ICD-10 coding procedures.
- Excellent communication skills, both verbal and written, with patients, healthcare providers, and team members.
- Ability to maintain composure and effectiveness in a fast-paced work environment.
- Strong interpersonal skills and a commitment to delivering high-quality healthcare.
- Adherence to professional standards, including safety protocols, punctuality, and appearance.
- Experience with remote work and safeguarding patient confidentiality in a secure workspace is essential.
Qualifications
- Certification: Must hold certification as a Medical Assistant through the AAMA or NCCT, or be willing to obtain certification within one year.
- Education: High school diploma or GED is required.
- Experience: A minimum of 3 months of experience in a medical office setting is preferred.
